Building policies

Policies and responsibilities

Select a section to read the policy. These standards are intended to support safe care and clear responsibilities for clinics, practitioners and the building.

01 Licensing and Health Directorate Approval

Each clinic or medical center must obtain the applicable authorization and approval from the relevant health authorities, including the Sulaymaniyah Health Directorate, before commencing services where such approval is required.

  • Practitioners must maintain valid professional registration, licenses and credentials applicable to their services.
  • Clinics must operate only within their approved specialty, scope of services and permitted facility category.
  • Required licenses and approvals must be renewed and kept valid according to the applicable rules.
  • The building's permission to use its premises does not replace any individual clinic or practitioner license.
02 Professional Qualifications and Clinical Responsibility

Chara Medical Building is intended to accommodate experienced specialists, including senior doctors and consultants. Professional qualifications and practice rights must be supported by the required documentation and approvals.

  • Each practitioner is responsible for decisions made within their professional scope and clinical competence.
  • Practitioners must maintain appropriate records and follow applicable professional and ethical standards.
  • No practitioner may provide a service for which they lack the required authorization, competence, staff or facilities.
  • Practitioners must communicate material risks, available options and follow-up instructions to patients.
03 Minor Procedures and Major Surgery Referrals

Clinics within the building may perform only minor surgical procedures that are permitted by their approvals and suitable for the clinic's facilities, equipment, staffing and clinical capabilities.

  • A procedure must not be performed in the building if it requires an operating theatre, inpatient care, specialist support or other hospital-level resources that are not available and authorized at the clinic.
  • Major operations and procedures beyond the clinic's permitted scope must be referred to an appropriately equipped and authorized hospital.
  • The treating practitioner is responsible for assessing clinical suitability, explaining the referral and arranging appropriate documentation and continuity of care.
  • In an emergency, staff must activate appropriate emergency assistance and arrange transfer to a suitable hospital when required.

The term “minor procedure” does not itself authorize a procedure. The applicable license, clinical standards and facility capabilities determine what may be performed.

04 Building Facilities and Equipment

Chara Medical Building provides the agreed premises, clinical environment and necessary equipment and facilities to support the authorized work of participating practitioners.

  • Equipment supplied by the building must be used appropriately and kept in suitable working condition.
  • Practitioners must check equipment suitability before use and report faults, damage or safety concerns promptly.
  • Additional specialty-specific equipment, supplies, maintenance or staffing responsibilities must be clarified between the building and the clinic before services begin.
  • Services must not proceed if essential equipment, supplies or safe operating conditions are unavailable.
05 Patient Safety and Infection Prevention

Clinics and practitioners must follow applicable infection prevention, sanitation, sterilization and patient safety requirements for the services they provide.

  • Hand hygiene and appropriate protective equipment must be used according to the clinical task and relevant guidance.
  • Instruments must be cleaned, disinfected or sterilized as appropriate to their intended use.
  • Clinical waste and sharps must be handled and disposed of using applicable healthcare waste procedures.
  • Suspected safety incidents, equipment failures and infection risks must be addressed and reported through the appropriate channels.
06 Patient Rights, Consent and Confidentiality

Patients must be treated with dignity, respect and professional care without inappropriate discrimination. Their privacy and confidential information must be protected.

  • Practitioners must obtain informed consent where required and explain relevant procedures, risks and alternatives.
  • Patient records and personal information may be accessed or shared only as permitted by applicable law and for legitimate care or operational purposes.
  • Patient information must not be disclosed casually or used for publicity without an appropriate lawful basis and any required consent.
  • Patients should be given appropriate instructions about treatment, medicines, follow-up and when to seek urgent care.
07 Clinic Operations and Professional Conduct

Clinics must operate responsibly and maintain professional standards consistent with their approved activities and the building's operating arrangements.

  • Practitioners and clinic staff must act professionally and treat patients, colleagues and building personnel respectfully.
  • Clinic activities must remain within the approved services and agreed use of the premises.
  • Clinical records, equipment and shared areas must be managed responsibly.
  • Changes to the services offered, clinic arrangements or responsible practitioners must be reviewed for any required approvals before implementation.
08 Referrals, Emergencies and Continuity of Care

Patients whose conditions exceed the clinic's approved capabilities must be directed to an appropriate healthcare facility without avoidable delay.

  • Practitioners must recognize when specialist hospital assessment, surgery, monitoring or admission is required.
  • Referrals should include relevant clinical information needed for safe and timely onward care.
  • Emergency situations must be managed within the practitioner's competence while appropriate emergency assistance and transfer are arranged.
  • Follow-up responsibilities and patient instructions should be communicated clearly.
09 Compliance, Inspections and Corrective Action

Clinics and practitioners must cooperate with applicable regulatory inspections and maintain the documentation required for their licensed activities.

  • Required licenses, professional credentials and relevant clinical records must be made available to the appropriate authority when lawfully requested.
  • Identified safety or compliance issues must be addressed promptly and within the required timeframes.
  • Activities must be restricted or suspended where necessary to address a serious safety risk or comply with a lawful regulatory direction.
  • The building may review use of its facilities in response to substantiated safety concerns, subject to applicable agreements and law.

Important: Clinical scope and patient safety

No clinic may exceed its approved scope of practice. Major operations and any procedure requiring hospital-level facilities must be undertaken at an appropriately authorized hospital. Patient safety takes priority over convenience or scheduling.
